Training Attestation Forms –
Southern California
Travelers, Registry Nurses & Nursing Students
will need to print the appropriate Attestation Form for your role BEFORE starting the lessons. Once you have completed all
the assigned lessons, sign the form and turn it in to your agency
representative or instructor.
Note that students should only use functionality
that is within their scope of practice regardless of what is reviewed in a
lesson.
